privacy & confidentiality
Your privacy and confidentiality are crucial to our work together, as they impact your trust and participation in treatment. As a clinical psychologist, my work is regulated by a clear set of professional, legal, and ethical requirements. I take this matter seriously, and have a strong privacy policy in place.
- I personally receive all calls, voicemail and emails with no additional staff.
- Records are not accessible to anyone else in my office. I require your written authorization before I release any information.
- I will not even acknowledge that you are a patient of mine without your signed authorization form.
- If I see you in the community, I will not acknowledge you unless you initiate it.
- I will not list any diagnosis in my records unless you choose to file for insurance reimbursement.
Limits of Confidentiality: I am legally obligated to breach confidentiality under the following conditions:
• Suspected child, elder or dependent adult abuse
• Threat of serious bodily harm to a person by the patient
• Intention to self harm
• If I cannot get a court order or subpoena cancelled

Other insurance carriers:
- I would gladly help you file for fair and accurate reimbursements for psychotherapy. My fees are considered within or close to the “usual and customary” (UCR) range and qualify for reimbursement under most insurance plans.
- Call your insurance company and find out your coverage percentage
- Also, ask about how they calculate your reimbursement
- You would be submitting claims for the CPT code 90837 (Individual Psychotherapy, Outpatient, 60 minutes), provided by a non-participating/out-of-network licensed psychologist, practicing in the 60523 zip-code.
- Turkish speaking clients might qualify for an in-network reimbursement if their insurance carrier does not have a Turkish speaking licensed psychologist on their panel.
Current out of pocket fee is $200 for the initial sessions and $150 per session thereafter.

cancellation policy
Please cancel your appointment at least 24 hours in advance of your scheduled session time. Insurance companies do not pay for missed appointments and my clients are responsible for the full session fee of $150 if they cancel after the 24-hour cancellation period. Please contact me to reschedule sessions in case of inclement weather that makes travel hazardous.
